1: Registration of death within 30 days

• If the death occurred in hospitals, health centers and dispensaries, ensure you obtain Burial Permit. In case of a death occurring at home report to Village Executive Officer or the District Registrar of Birth and Deaths to obtain burial permit. This should be done within 30 days.
• Log in to the eRITA system https://erita.rita.go.tz/auth choose the language you like to use and then register to get an account which will enable you to log into the system
• Log in to the eRITA system and then click death services then go ahead click on new certificate and then fill in the deceased information correctly
• Make sure you scan the death notification then attach it after filling in the deceased information.
• The system will generate control number of Shs.8,000   use it to make payments in mobile networks by choosing government services
• After completing the application, you are advised to visit your account to check if it has been approved, if you see the word approved or issued, know that your certificate is ready and if you are given any instructions, kindly act on them.

2: Late registration of deaths.

• Log in to the eRITA system https://erita.rita.go.tz/auth choose the language you like to use and then register to get an account which will enable you to log into the system
• Log in to the eRITA system and then click death services then go ahead click on new certificate and then fill in the deceased information correctly
• Make sure you scan the death notification and Submit supporting documents (Minutes of the family meeting and Letter from relevant government authorities such as Ward Executive Officer, Village executive officer) to prove death occurance then attach them
• The system will generate control number of Shs.20,000   use it to make payments in mobile networks by choosing government services
• After completing the application, you are advised to visit your account to check if it has been approved, if you see the word approved or issued, know that your certificate is ready and if you are given any instructions, kindly act on them.

3: Certification of death certificates

• Log in to the eRITA system https://erita.rita.go.tz/auth choose the language you like to use and then register to get an account which will enable you to log into the system
• Log in to the eRITA system and then click death services then go ahead click on verification and then fill in the deceased information correctly
• Make sure you scan the death certificate
• The system will generate control number of Shs.7,000   use it to make payments in mobile networks by choosing government services
• After completing the application, you are advised to visit your account to check if it has been approved, if you see the word approved or issued, know that your certificate is ready and if you are given any instructions, kindly act on them.
  

4: Registration of foreign death

• Submit  the death certificate ( from the country death occur)
• Letter from embassy to notify RITA of the event occur in the particulary country
• Medical certificate.
• Pasport of deceased
• ID of informat
• Pay appropriate fee ( current fee rate Tshs 30,000/=).

5: To make correction on the certificate of death

• Submit request for correction
• Attach supporting documents
• Surrender certificate which needs to be corrected.
• Pay appropriate fee if application allowed( current fee rate Tshs.13,000/=)
